South East Grey Community Health Centre (SEGCHC) opened its doors in October, 2011 at the Markdale site. We have satellite offices at the high school in Flesherton, a physician office in Dundalk and at the municipal building in Chatsworth.
SEGCHC is a Not for Profit organization which achieved Charitable status in April 2014. It is governed by a Community Board and an Accountability Agreement with the South West Local Health Integration Network – our funders. The SEGCHC receives 96% of its operating funding from the provincial government. Revenues for 2016-17 were 3.4 million and it has $.6 Net capital assets.
The SEGCHC provides primary healthcare and community development services to a rural population comprised of approximately 35,000 residents of South East Grey which encompasses the communities of Southgate, Grey Highlands, West Grey and Chatsworth.
We have a current patient roster of 5,000 patients and through our inter-disciplinary team of professionals, also provide allied health support to an additional 700 patients referred through a partnership arrangement with local sole practitioners.

The SEGCHC has four Specialists working in-house:
Psychiatrist
Neurologist
Internal Medicine Specialist
Pediatric Clinical Psychologist

In 2015-2016 the SEGCHC received a 99.2% patient satisfaction rating from an independent third party survey of our patients – the highest patient satisfaction rating in the SW LHIN.
The SEGCHC is operating a Healthy Kids Program in collaboration with the Municipality of Grey Highlands, funded through the Ministry of Health for a three-year period with annual operating revenues and expenses of $125,000. This program is a three-year commitment and provides services to youth 5 years to 13 at ten different primary school locations in South East Grey with a focus on improving health through increased exercise (activity) and improved diet.
In 2016-17 we were awarded an Ontario Trillium Grant of $500,000 which will be utilized for expansion of programs and capital. These funds will be flowed over the next three years.
During 2016-17 we were awarded a Community Infrastructure Grant of $100,000 for capital improvements to the building.
