Toronto, Ont —Looking for a family physician to take over a practice of 1,100 (1,000 rostered) patients at Taddle Creek FHT in Toronto, steps from the Christie subway station. The office is shared with 2 other physicians and a NP, and we are part of a larger network of 18 family doctors. Our office has dedicated and competent administrative staff. We have used an EMR (Telus PSS) for 18 years, and patient charts are well maintained. There is a lab and imaging in the building. The practice is made up of a diverse group of patients ranging in age from 0 to 100. Routine screenings are up to date. We are an FHO/FHT so patients have excellent access to a variety of programs and allied health professionals including pharmacy, social work, dietitian, and nursing support; diabetes education; and psychiatric consultation.
