Family Physicians Opportunities - Carefirst Family Health Team
The Carefirst Family Health Team (FHT) is seeking full-time/part-time permanent and locum family physicians to join our innovative, interdisciplinary team dedicated to delivering high-quality, community-based care.
About Us
Carefirst FHT is committed to delivering exceptional, comprehensive primary care through a collaborative, team-based model that includes family physicians, nurse practitioners, nurses, social workers, service navigators, dieticians, physiotherapists, and medical receptionists. We have a robust Chronic Disease Management and Prevention program, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives.
Our “Hub & Spoke Model” and strong community partnerships ensure coordinated, high-quality, patient‑centred care across the continuum. We work closely with Carefirst Seniors & Community Services Association to provide truly integrated services.

•Ontario Health atHome (formerly Home and Community Care Support Services)
•MINT Memory Clinic
•Ontario Structured Psychotherapy (OSP) Program
•Specialist Clinics: psychiatry, cardiology, endocrinology, optometry, geriatric medicine, gynecology, chiropody, rheumatology, physiotherapy, and audiology
Clinic Locations
We currently operate four well-established clinics across Scarborough, Markham and Richmond Hill, and continue to expand in Markham to support continued growth and community needs.

•420 Hwy 7 East, Richmond Hill (West Beaver Creek / Hwy 7)
•360 Highway 7 East, Richmond Hill (Highway 7 / Chalmers Rd)
•105 Gibson Centre, Markham (Steeles Ave East/Warden Ave)
Upcoming new clinic locations:
•Unionville Commons Seniors Hub, Markham (Unionville Main Street/Highway 7 East)
Physicians will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.
The Position
With this rapid expansion, we are looking for compassionate, team-oriented family physicians who are committed to delivering high-quality care and value interdisciplinary collaboration to fill a new position at our Family Health Team.
An existing patient waitlist offers an excellent opportunity to establish a thriving practice quickly. We welcome new graduates and international medical graduates fulfilling return-of-service requirements!

Compensation
▪Income stabilization program option, fully funded by MOHLTC
▪Blended Salary Model position with benefits
▪Vacation Leave, Sick Leave, Flex Day and Continuing Medical Educational Leave for FTP/PTP position
▪$170 per hour for Locum position
