that serves over 18,000 clients annually. Established in 1978, SWCHC provides a wide range of
health care services such as primary health care, health promotion, community development, mental

SWCHC strives to ensure that our team reflects the communities we serve. We work within an
anti-racist, anti-oppressive framework that acknowledges the existence and impacts of systemic
racism and seeks to ensure that systems are fair and equitable for everyone.

enhance health outcomes by implementing culturally responsive, patient-centered interventions right
within the heart of refugee communities. The aim is to ensure improved access to a range of
healthcare services, focusing on key areas to improve access to healthcare services, including
primary care, mental health and trauma counselling, pediatric care, immunization and infectious
disease care, and women’s health care. A culturally responsive approach will
be utilized including access to interpretation services, and system navigation support. As part of this specialized team, the physician will have the opportunity to work within an
interprofessional
team of physicians, nurse practitioners, nurses, psychotherapists, community health workers,
interpreters, and health promotion staff. The specialized team will medically stabilize clients and
facilitate transfer of the client to the community and act as a clinical resource to primary care
providers & organizations following transition.
