Country Roads Community Health Centre is a not-for-profit organization funded by Ontario Health and the Ministry of Health. Since 1988, we’ve provided equitable, client-centred care to rural communities in Eastern Ontario.

Our interdisciplinary team offers services such as primary care, chronic disease management, health promotion, system navigation, and outreach—working with community partners to meet local needs and support healthier communities.

The Opportunity

Country Roads Community Health Centre (CRCHC) is seeking a Family Physician to join our interdisciplinary primary care team serving the rural communities of Rideau Lakes, Westport, Leeds and Thousand Islands, and surrounding areas.

As a Community Health Centre, we deliver care through an integrated, multidisciplinary model that brings together physicians, nurse practitioners, nurses, social workers, dietitians, pharmacists, community health workers, and health promoters to address the full spectrum of an individual's health and wellbeing. Key Responsibilities

  • Provide comprehensive primary care services to a rostered patient population of up to 1130 patients, prorated to FTE.
  • Diagnose, treat, and manage acute and chronic medical conditions
  • Collaborate with interdisciplinary team members to develop and implement care plans
  • Support chronic disease prevention and management initiatives
  • Shared on-call services amongst primary care provider team
  • Participate in quality improvement and organizational planning activities
  • Promote equitable access to care and address social determinants of health
  • Maintain accurate clinical documentation and adhere to professional standards

Qualifications

  • Current and unrestricted registration with the College of Physicians and Surgeons of Ontario.
  • Current registration with the College of Family Physicians or equivalent experience
  • Demonstrated ability to work effectively in a multi-disciplinary team environment
  • Excellent health assessment and clinical skills
  • Demonstrated commitment to and knowledge of community-based health care
  • Sensitivity to people with a variety of health, social, cultural and economic needs
  • Excellent communication skills, both oral and written
  • Excellent organizational and interpersonal skills
  • Experience and understanding of rural communities and rural health needs

Compensation & Benefits

Compensation: $182.40/hour or $331, 968 annually for full time.

Schedule: up to 35 hours/week, flexible schedule, on-call
