
PHYSICIAN
Key areas of responsibility:
Provide full-scope, patient- and family-centered care, with focus on clients with multiple
chronic conditions, mental health, addictions, and social vulnerabilities.
Lead and support complex care management, including ongoing case planning for high-
intensity clients.
Offer clinical leadership through consultation, mentorship, and development of medical
directives and care pathways.
Advocate for clients facing barriers such as poverty, housing insecurity, and access to
medications.
Collaborate with the interprofessional team and external partners to ensure coordinated,
holistic care.
Participate in health promotion, screening, and outreach initiatives tailored to underserved
populations.
Practice using trauma-informed, harm-reduction, and culturally safe approaches.
Supervise and mentor medical and NP learners, contributing to teaching and professional
development.
Engage in quality improvement, research, and evaluation to strengthen client outcomes and
reduce inequities.
Provide on-call consultation to Nurse Practitioners and participate in extended hours/after-
hours coverage
