PHYSICIAN

Key areas of responsibility:
 Provide full-scope, patient- and family-centered care, with focus on clients with multiple
chronic conditions, mental health, addictions, and social vulnerabilities.
 Lead and support complex care management, including ongoing case planning for high-
intensity clients.
 Offer clinical leadership through consultation, mentorship, and development of medical
directives and care pathways.
 Advocate for clients facing barriers such as poverty, housing insecurity, and access to
medications.
 Collaborate with the interprofessional team and external partners to ensure coordinated,
holistic care.
 Participate in health promotion, screening, and outreach initiatives tailored to underserved
populations.
 Practice using trauma-informed, harm-reduction, and culturally safe approaches.
Supervise and mentor medical and NP learners, contributing to teaching and professional
development.
 Engage in quality improvement, research, and evaluation to strengthen client outcomes and
reduce inequities.
 Provide on-call consultation to Nurse Practitioners and participate in extended hours/after-
hours coverage
