Our interdisciplinary team including physicians, nurse practitioners, registered nurses, physiotherapists, occupational therapists, dietitians, and massage therapists works together with one clear mission: to keep residents healthy, active, and comfortable in their own homes.
Position Summary
Perennial is hiring an In-House Physician to provide on-site primary and urgent care services at a partnered retirement residence in Ottawa, Ontario, one to four times a week.
The residence is home to approximately 200 residents, with a mix of independent living and assisted living. Residents are generally mobile, cognitively well, and actively engaged in their community, while many manage common chronic conditions such as hypertension, diabetes, chronic pain, COPD, cardiovascular disease, and mild cognitive decline.
The physician will have the opportunity to develop a roster of interested residents and, where applicable, retirement residence staff, providing ongoing primary care and urgent assessments. You will work closely with an on-site Nurse Practitioner and Care Coordinator, as well as Perennial’s broader interdisciplinary clinical team.
This is an excellent opportunity for a physician who enjoys building meaningful relationships with older adults and wants to practice in a collaborative, community-based environment with minimal travel and strong clinical support.
Key Responsibilities
Primary & Urgent Care
-Provide full-scope family practice and urgent assessments to rostered and non-rostered residents and staff.
-Provide longitudinal care for chronic conditions commonly affecting older adults.
-Determine when escalation to emergency, hospital, specialist, or other community services is required.
Resident Rostering & Continuity of Care
-Enroll interested and eligible residents into your practice roster, where appropriate.
-Establish ongoing physician-patient relationships and provide continuity of care.
-Collaborate with residents, families, substitute decision-makers, and residence staff as appropriate.
-Support transitions of care following hospitalizations, emergency department visits, or specialist consultations.
Geriatric & Preventive Care
-Provide evidence-informed care tailored to the needs of older adults.
-Conduct medication reviews and identify opportunities to optimize medication use.
-Support chronic disease management and preventive health initiatives.
-Assess falls risk, functional decline, cognitive concerns, nutrition, mobility, and other factors affecting seniors' health and independence.
-Work collaboratively with allied health professionals to support resident wellness and quality of life.
Interdisciplinary Collaboration
-Work closely with Perennial’s Nurse Practitioners, Registered Nurses, Care Coordinators, and allied health professionals.
-Participate in case discussions and care planning for residents with complex or evolving needs.
-Contribute to clinical protocols, quality-improvement initiatives, and the development of Perennial’s care model in Alberta.
Virtual Care & Documentation
-Use Perennial’s EMR and virtual-care tools, or an approved physician EMR where applicable.
-Complete timely clinical documentation, orders, referrals, and follow-up.
-Communicate with families, caregivers, community providers, and other members of the care team as clinically appropriate.
Utilize virtual care when appropriate to supplement on-site services and improve continuity of care.
Qualifications
-Licensed for independent medical practice in Ontario and in good standing with the College of Physicians and Surgeons of Ontario (CPSO), with an active practice certificate appropriate to the role.
-Eligible to submit claims to the Ontario Health Insurance Plan (OHIP) for eligible insured physician services, in accordance with applicable Ontario legislation and the OHIP Schedule of Benefits.
-Eligible for and maintaining appropriate Canadian Medical Protective Association (CMPA) or equivalent professional liability coverage for the scope of practice.
-Minimum two years of experience in family medicine, primary care, geriatric medicine, or complex/chronic-care settings.
-Demonstrated ability to assess acute changes in condition, recognize clinical deterioration, and determine when escalation to emergency, hospital, specialist, or other community services is required.
-Experience caring for older adults and patients with complex or multiple chronic conditions is strongly preferred.
-Comfortable practising independently while collaborating with on-site and virtual Nurse Practitioners, Registered Nurses, Care Coordinators, and other interdisciplinary team members.
-Comfortable assessing and managing common acute and chronic medical conditions in a retirement-residence setting.
-Experience with or comfort managing common geriatric issues, including polypharmacy, medication reconciliation, falls, frailty, cognitive impairment, delirium, functional decline, and chronic pain.
-Comfortable communicating with residents, families, substitute decision-makers, caregivers, and retirement-residence staff regarding treatment plans, changes in condition, and goals of care.
-Proficient with electronic medical records (EMRs) and virtual-care platforms.
-Demonstrates excellent communication skills, bedside manner, sound clinical judgment, and a strong commitment to relationship-based, patient-centred care.
Compensation & Schedule
-Stipend provided for the In-House Physician role.
-OHIP fee-for-service billing applies to eligible insured physician services provided to eligible Ontario residents, subject to the applicable Ontario legislation and OHIP Schedule of Benefits.

-Minimal travel and administrative overhead compared with a traditional community practice.
