Job Description
In partnership with Inner City Health Associates (ICHA) and Shepherd Village Retirement Home, Scarborough Health Network (SHN) is shaping the future of care by creating a new primary care service to meet the health care needs of Scarborough.
Each of the three organizations provide many programs and services designed around the needs of one of Canada’s most vibrant and diverse communities. Shepherd Village is Toronto’s largest not-for-profit seniors community committed to meeting the changing physical, spiritual and social needs of seniors through quality care and compassionate service. SHN is home to North America’s largest nephrology program, as well as the designated cardiac care and spine centre for Scarborough and surrounding communities to the East. Inner City Health Inner Associates is the largest homeless health organization in Canada, offering Toronto’s homeless and precariously housed population with direct access to high-quality health services. Together, we are proud to be a teaching site for the University of Toronto and partner with a number of other universities and colleges, helping to train the next generation of health care professionals.
SHN, ICHA and Shepherd Village have developed a unique and exciting collaborative program to provide excellent primary care for vulnerable and complex client populations in Scarborough. The SHN Primary Care Clinic will be located at the Shepherd Village Retirement Home to provide primary health services to the residents and staff of Shepherd Village. Successful candidates will establish a comprehensive care family medicine practice with access to a wide-range of subspecialists at SHN. Incentives will include subsidized overhead and staffing support. ICHA primary care clinics take place at a number of shelter sites in Scarborough and be supported by ICHA’s wide network of outreach specialists with virtual office administrative supports.
We are looking for 2 Family Medicine Physicians with an interest in working in this clinic to provide primary care for seniors, children, adults, women, LGBT+ and vulnerable complex patients. Priority will be given to candidates with a special interest in community and transitional primary care and/or population health, health equity and with a diversity focus. Additional opportunities to participate in resident teaching, research, and community activities can also be explored.
Job Requirements
Successful candidates must be licensed or eligible for a license to practice in Ontario with accreditation in Family Medicine by The College of Family Physicians of Canada (CCFP). They will also be required to apply for an appointment in the Department of Family and Community Medicine and Courtesy privileges at SHN. Excellent interpersonal skills and a commitment to inter-professional care are essential.
