North Lanark Community Health Centre (NLCHC) is seeking a dynamic Primary Care Physician to join our interprofessional team. This is a contract position from July 23rd 2018 to August 26th 2019, covering a one year sabbatical. In addition to providing primary care in a multi-disciplinary setting, this position includes responsibilities for telephone on-call on a rotation basis. Role: The Primary Care Physician (M.D.) functions as a member of the primary health care team in the provision of primary health care services offered at North Lanark Community Health Centre (NLCHC). The M.D. will provide on-call services to clients of NLCHC as well as urgent care. Services include primary health care with an emphasis on health promotion, client education and self-management. The Physician must be aware of, understand, and adhere to the Standards of Practice set out by the College of Physicians and Surgeons of Ontario.

1. Provides ongoing, comprehensive clinical services to clients either at NLCHC or in the community, taking into account the environmental, social, psychological and medical factors which influence health.

2. Performs clinical duties by seeing and assessing clients for routine health and illness care and prescribing treatment as required. Services are provided at NLCHC and in the client’s home, if required.

3. Participates in the on-call rotation through the after-hours on-call system for the CHC group.

4. Provides urgent/emergent care when required.

5. Encourages clients and their families to take responsibility for their own health by involving them in risk factor and health problem identification, goal setting, and the choice of interventions for disease treatment and prevention and health promotion.

6. Ensures appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records for all clients, participating in chart reviews, discussion of complex cases, and referring clients to other health professionals and programs both within NLCHC and in the community.

7. Promotes an inter-disciplinary approach to the provision of care.

8. Providing appropriate support and consultation to other staff within NLCHC and participates in staff and team activities as required.

9. Facilitates the administration of the Centre by identifying areas where the development of protocols and procedures is needed to ensure high quality services or to promote effective team functioning.

10. Assists with the overall planning, development and evaluation of health services by identifying health needs in the community and working with other team members to develop methods for program evaluation.

11. Plans and participates in outreach activities related to primary health care.

12. Initiates and maintains functional relationships with relevant community professionals, agencies and services.

13. Performs other related duties as requested.

 Submits records of time worked and time off in a timely manner

 Maintains and develops professional competence, and a professional licence to practice, through appropriate continuing education methods (e.g. CME, peer interaction, literature review, conferences, courses, staff development leave, etc.)

Job Requirements


Education  Current and unrestricted registration with the College of Physicians and Surgeons of Ontario.  Current registration with the College of Family Physicians or equivalent experience 2. Professional Experience  Demonstrated ability to work effectively in a multi-disciplinary team environment  Excellent health assessment and clinical skills.  Demonstrated commitment to and knowledge of community based health care.
