Family Physician

28 Hours per Week

Stonegate CHC has been providing primary health care, health promotion and a wide variety of community programs to the South Etobicoke Community since it was created by community members in 1992. Our Mission is to improve the well-being of our community. Together, we identify individual and community needs and respond by providing accessible and inclusive health services and health promotion programs. We envision a healthy, active and inclusive community where physical, mental and social well-being is achieved.

We are currently recruiting a permanent, part time (28 hours per week) Family Physician. We are seeking a Family Physician with strong clinical skills with a commitment to health equity and team based care. The Family Physician will join our Clinical team staffed by other physicians, nurse practitioners, nurses and administrative support. The Family Physician will work as part of an inter-professional team to provide primary health care to clients within a care model which emphasizes health promotion, client education and makes special efforts to encourage the participation of individuals and groups facing barriers to achieving health equity. The Family Physician will work collaboratively with several allied health professionals as well as our health promotion facilitators.

Job Types:  Permanent, Part Time

Salary Range:   $232,326 - $285,235 plus on-call stipend (pro-rated to $185,860 - $228,188 for .8 FTE)

Benefits: We provide a generous benefit package including paid vacation and professional development time, extended health benefits and a HOOPP Pension plan.

Hours: Regularly scheduled hours, 28 hours per week including evening hours. Participating in an on-call rotation approximately once every 5 weeks.

Job Requirements
Key Qualifications:

· Medical degree from a recognized university

· CCFP Certification or equivalent practical experiences

· Demonstrated ability to coordinate care collaboratively with other inter-professional team members

· Demonstrated ability to relate therapeutically with a diverse client base

· Excellent communication, decision-making, problem-solving, conflict management and time management skills

· Proficiency in the use of computers and software applications related to primary health care. (We use Telus PS Suite for our electronic medical record).

· Second language is an asset
