About the Department

Sunnybrook’s Department of Family and Community Medicine was one of the first academic programs of its kind in Canada and was the first academic family medicine department at the University of Toronto. The Department has a rich history and a bright future. It is divided into five divisions: Family Practice Unit, Long Term Care, Palliative Care, Family Practice Obstetrics and St. John Rehab Hospitalists. What is the Department’s role at Sunnybrook?

Sunnybrook’s Strategic Plan emphasizes the essential role the hospital plays, not only through its tertiary regional and provincial programs, but also supporting the community needs in North Toronto. The Department of Family and Community Medicine is a vital link that connects the community and the hospital. It works to create a network of primary care physicians and institutions that enable a seamless tether to a continuum of care for patients.

 

What will success in this role look like?

Sunnybrook has a lofty vision to invent the future of health care. The Hospital’s leaders are expected to motivate their teams to achieve this vision through the implementation of Sunnybrook’s Strategic Plan. In this role, the successful candidate will build partnerships with care providers and agencies in the many communities we serve to support a truly integrated system of patient care. The Chief will continue to create a high performing team and be a strong mentor to trainees and faculty at all stages of their careers. The successful candidate will lead the team to develop a track-record of peer-reviewed research that is innovative and aimed at improving care.

 

Job Requirements

Qualifications

Candidates must hold an MD and have certification from the College of Family Physicians of Canada, or equivalent qualifications and will be eligible to hold a clinical full time academic appointment at the rank of Associate or Full Professor in the Department of Family and Community Medicine at the University of Toronto.
